About UsInitiatives & ProjectsPalliative Care Consultation TeamsThe Palliative Care Consultation Teams (PCCT) are funded through Aging at Home funding from the ESC LHIN. Application and advocacy for these teams was done through the ESC EOLCN. The ESC EOLCN serves in an advisory role for these teams.
